diabetes, high blood pressure, short cervix, low fluid volume, decreased fetal movement & bleeding.
Initial Visit Preferred Prior To the 12th Week Gestational Age
- Discuss diet, smoking, alcohol and medical and obstetric histories
- Discuss genetic screening. Address nausea and vomiting
- Ultrasound may require return visit depending on insurance and gestational age
Physical Exam and Complete History Typically 12th Week
- Physical exam
- Review of medical, surgical, obstetric and family history
- Pap smear and STD screen
16 & 18 Weeks Gestational Age
- Ultrasound for cervical length
- Influenza vaccine offered, depending on time of year
20 Weeks
- Ultrasound for cervical length and fetal anatomic survey
24 and 28 Weeks
- Measure fetal heart rate and fundal height
- dTap (diphtheria, tetanus and pertussis) vaccine offered after 26 weeks
- Screen for gestational diabetes at 28 weeks
- Rhogam given if Rh negative blood type
30 -36 Weeks
- Measure fundal height and fetal heart beat every 2 weeks
- Group B strep and STD screen at 34 weeks
37 – 40 Weeks
- Weekly fetal heart rate and fundal height measurement
- Cervical exam at 39 weeks & discuss delivery
- Induction offered at 40 weeks if cervix short, soft & partially dialted
- If indicated, C-section at 39 – 40 weeks
- For twins, deliver at 38 weeks
40 – 41 Weeks
- Ultrasound for measurement of amniotic fluid volume
- Monitor fetal heart beat pattern every 3-4 days
- If not delivered by 41 weeks, induction recommended
